What makes a company go from just mediocre and good to Great?

In today's society and high paced environment companies are open 24/7 but at what price? I can't tell you how many times I've called a company who I've been used to their excellence customer service to only hang up completely disappointed. The company that once was ranked high was now on the bottom of my list because of their customer service and responsiveness to my concerns. Which once used to be a phone call and a live representative turned into a phone call with 5 different transfers and a representative that was not empowered to make decision. Companies might think they are being proactive by serving their customers at all hours of the night but if they are not able to train the company culture and train their extended hours answering service what customer service is expected from their customers they loose a customer.
